Same woman plus 50 lbs is just gonna be less attractive. Not judging her character or any of the esoteric shyt. You're just gonna prefer the in shape version.
And that's basically the issue. We watch movies and wrestling and shyt to see attractive people doing interesting things.
You are surrounded by fat lazy people at every turn. There's no reason for you to tune in to see people that look like your frumpy neighbors.
I believe fat people themselves know they look better when they lose weight. I am no spring chicken and I got a beer belly. When I see myself I cringe a little and remember back to having abs and shyt. No illusions for me.

America has a childhood obesity problem, so until that's solved, there will be/are millions of overweight to obese little girls who want to see a short movie with a protagonist that looks like them.
