HIV/AIDS rates for vaginal sex is extremely rare in developed countries. If you aren't in Russia or [insert African country] you could have unprotected sex with a woman who has HIV and not get it. I wouldn't advise that obviously but it's just an example of how low the rate is.
No reason for a heterosexual man to use that medication, even if you live in Atlanta.
Wouldn't Thailand, Colombia, DR, Philippines, etc. also be on that list? Also, why Russia? I never heard that rates were crazy out there.

Okay, these are the kind of things that I would be concerned with. Obviously, no one is trying to hook up with a chick that is HIV positive, but if you didn't know before sleeping with her that she had been living that polyamorous life where she might have been sleeping with other polyamorous people (including down low men), then I thought it might be a worthwhile drug to consider.
If it messes up your immune system though, then I can't see it being worth it though, and that you'll probably be fine as long as you do your due diligence beforehand.
