Do you think doctors sometimes kill people on purpose?

I feel like they do. If the family says to operate and the doctor doesn't think it's a good idea they just let them die in surgery to not have to deal with the patient anymore
Too many people in a hospital for that. They and their hospital systems are scared shytless of lawsuits. Docs will check other docs for malpractice. So will NP(s), physician assistants, and your longer term nurses. During her medical residency 1,2, and 3-years, my aunt says that the old nurses would brutally and repeatedly check her for random sht that wasnt by-the-book.

Beyond a few outliers, I think just the high tier specialties like nuero, or transplant, or orthopedics would be susceptible to intentional malpractice because few folks in a hospital can keep up with the intricacies of what they do.

Depends on what type of killer they are. What you are describing is complicated and will get them discovered very quickly.
