Looks like a keratin accumulation. We have stuff like that all over our skin and it's mostly in nails.
I wouldn't be surprised if it affects mostly women because it's women who have the most trouble with their hormones.
Maybe it's autoimmune because keratin is created to protect the skin from stress.
