Still hasn't happened and you're crazy if you aren't using 2FA on any account you value. The only problem with 2FA now is hackers are social engineering their way into getting people's cell phones.
For example if you have an online account with certain cell phone providers if a hacker can get that password they can move your cell phone service to any other phone they want and you couldn't stop it. Some of them call in and pretend they work at a store and get your info that way. One chick had had cable provider give her info over the phone to someone else.
