I was born in the 80's in a developing nation... i was there at a critical time to see what childhood illness looked like before and after mass vaccination. To see the effects it had on those less fortunate to not get inoculated at young ages. Their scars and disfiguration. Damaged for the rest of their life and reduced quality of life. All the ones who never made it past childhood.

Good luck to the fools who hate education. You must learn.

Below is a full report detailing the infant mortality rate by racial demographics in the United States prior to widespread vaccination in the 1980s, including statistical trends, underlying causes, and public health implications.cdc+2

National Overview and Racial Disparities

  • In 1980, the overall infant mortality rate (IMR) in the U.S. was 12.6 per 1,000 live births.jamanetwork+1
  • Infants born to Black mothers experienced much higher mortality than those born to White mothers:
    • Black IMR: 22.2 deaths per 1,000 live births
    • White IMR: 10.9 deaths per 1,000 live birthscdc+2
  • The Black-White IMR ratio was about 2.0, meaning Black infants had twice the risk of dying before age one compared to White infants.cdc+2

Birthweight and Infant Mortality

  • Low birthweight (LBW) (<2500g<2500g<2500g) was a major factor in infant deaths; rates of LBW were twice as high in Black infants as among White infants, a persistent discrepancy linked to higher IMR for Black infants.jamanetwork+1
  • In 1980:
    • Black LBW rate: About 12.7%
    • White LBW rate: About 5.7%
  • For very low birthweight (VLBW) (<1500g<1500g<1500g): rates and associated mortality were again substantially higher for Black infants.cdc+1

Trends and Key Findings

  • From 1980 to 1991, the U.S. overall IMR declined by 29.4%(from 12.6 to 8.9), but the Black-White gap widened:
    • IMR for Blacks declined 20.7% (22.2 to 17.6)
    • IMR for Whites declined 33% (10.9 to 7.3)
    • The racial disparity ratio increased from 2.0 to 2.4 over the decade.cdc
  • Causes of infant death included complications of preterm birth, low birthweight, and infections; improvements in medical care benefited all races, but Black infants saw smaller proportional gains than White infants.jamanetwork+1
  • Geographic, socioeconomic, and access to health care factors amplified these disparities, with Black infants in many states—particularly in the urban South—having the highest risks.cdc

Table: Infant Mortality and Birthweight by Race, U.S., 1980

IndicatorBlack (1980)White (1980)
Infant Mortality Rate (per 1,000)22.2 cdc+210.9 cdc+2
Low Birthweight Rate (%)12.7 jamanetwork5.7 jamanetwork
Very Low Birthweight Rate (%)2.48 jamanetwork0.90 jamanetwork

Causes and Public Health Implications

  • Higher risk among Black infants was driven by both increased frequency of low birthweight/preterm delivery and higher mortality among all birthweight categories.jamanetwork+1
  • Underlying causes included disparities in maternal health, prenatal care, poverty, and environmental exposures.cdc+1
  • Despite medical advances, reduction in IMR was more rapid for White infants, leading to greater racial disparity by late 1980s.cdc

Conclusion

In summary, before widespread vaccination in the early 1980s, Black infants in the U.S. had more than double the risk of infant death than White infants primarily due to persistently higher rates of low and very low birthweight as well as sociodemographic inequities. While improvements occurred in overall infant mortality, the gap between racial groups persisted and, in some periods, even widened.cdc+2
