Read a report by The Informer a few years back that they use the godfathers old hat and still draw names and booking decides who throws who out
Bruce Prichard outlined it 100% on one of the podcast like 2 months ago.
They start with the finish, decide specific issues or conflicts to address, the stagger surprises, and work the entire order and when people go out. Like "road Dogg goes out at 6, and you get eliminated after triple H comes out" etc. The specific who attacks who is only outlined if there are rivalries to be outlined in the match. It's fully outlined though.

So what about surprise returns? Like when Cena came back in 2008? From what I know, none of the wrestlers knew he was going to be there. Do they just tell them someone's coming at #30 and leave it at that?
Sometimes it's misdirection instead of cena they may say "big show is last out" and by show might not even come out
Never mind the Royal Rumble...I want to know how in the hell did they organize the World War 3 battle royale. I guess James Barnett told everybody involved just to put Hogan over

As far as during the match, ringside refs and wrestlers are constantly communicating back and forth. Watch a recent rumble match and look at the guys laying on the mat, especially underneath or near the turnbuckles. Typically, you catch them getting directions from a ref while the ref is receiving directions from backstage through his earpiece.
@portcityplaya its the royal rumble 97 episode, about 2 hours in. The whole episode is great, one of their best relistening to it.
