Age between 21-39 years old
BMI between 19-29
Not underweight/under 100 pounds
No history of gastric bypass
Not currently on government financial support
Given birth to and be raising at least one child
Must provide OB/GYN records and a clearance letter
Records must show no history of the following: Hypermesis Gravidarum, Diabetes/Gestational Diabetes, Pre-Eclampsia, Placental Abruption, Preterm Labor/Delivery, Interuterine Growth Restriction, Recurrent Pregnancy Loss, Accreta, Post-Partum Hemorrhage, 4 or more previous c-sections, 6 or more pregnancies
Non-smoker living in a non-smoking home
◦Surrogate must be completely non-smoking for a minimum of 1 year before applying to the program
No history of criminal activity
◦Surrogate and partner must agree to criminal background check
Psychiatric Health
No history of clinical mental illness
No history of extreme post partum depression
Stable, responsible lifestyle
Surrogate and partner must agree to psychological testing
Must agree to home visit with all persons and pets residing in the home present
Financially sound
Surrogate and partner must agree to a credit check
Reside in a state with surrogacy-friendly laws
