This was towards the end of her life, somebody pointed out in the comments how she lost use of her right arm. It's in the same position throughout the video
...this song is just too sad to listen to, for me. once someone pointed her hand out and she goes on to sang the ad-libs "I don't want to go" and "Save me" at the end of the song--i just lose it. this song hasn't been the same for me since.
