Only happens when I'm logged in
Appears on threads I have posted on
I turned off add-ons, adblocking programs and reinstalled Firefox. It still appears.

Edit - Just happened to this thread. Its even more annoying because my name appears twice.

I see what the issue is. Your avi looks like a broken link. Fix that and then see if it works.
