Lose a substantial amount of body mass/fat in a relatively short period of time and you're gonna look kinda sickly initially, especially if you have expression lines. Doesn't help that he shaved off his beard. Most cats look WEIRD as fukk clean shaven if they've had facial hair for a long time

Lighting and color grading of that photo are adding to it, they make his skin look chalky, grey and lifeless. That's definitely part of it.
