Just your SSN, DOB, Address, Blood type, Ancestry results, dental records, car make model vin and license plate, street you grew up on, mother's maiden name, a copy of your fingerprints, last TB shot records, colonoscopy results (if applicable), phenotype, height weight and age, college transcripts, birth certificate, library card number, and any allergies to medications
