Cosa Nostra is really old, and started during the Middle Ages in Sciliy. The Italian mobsters were successful because they inherited a strict hierarchical structure that made them more competitive than other loosely organized outfits. They also had the advantage of being able to intermingle, bribe and extort the wider “white” society which black and latin organizations could not.

But yeah there were alot of stupid, meatheads in the mob that were carried by earners and leadership. It’s just the mob had the pedagogy, and structure in place to control their idiots unlike gangs which tend to be ran by them.
