After further review..

He did hit her hand but he also hit her face too.

Her head doing a "whole turn" is the exact result of getting smacked across the face unexpectedly. Despite the fact that she was taunting him, it can be assumed she still thought he might not do it.

Didn't notice him smiling, but if he was smiling its probably because of the laughter he heard around him. Smiles are also sometimes masks for fear.
