That’s why I stick to dumbbell bench press. Free range of motion. Straight bars restrict you too much and put too much strain on the bones. If your technique is slightly off it can be a wrap. Dumbbells are way more forgiving and adaptable.

What are the warning signs?Wouldn't be surprised if this was a pre fracture. Meaning he was ignoring pain and warning signs. Compound breaks rarely happen out the blue and when they do they're pretty much freak accidents weather it's weightlifting, basketball or MMA.
I disagree. I simply don't max out without a spotter and if I leave the clips off I can dump one side if I somehow did get stuck. I've been benching solo for 16 years lolDid they take all of the Smith machines out of gyms since my era?

But since you're not maxing out or going close to that, why not just use the Smith?
Or use dumbbells ?

Most gyms dont have dumbbells heavy enough once you start getting up there in bench plus they are a pain in the ass to set up.
