In the Attitude era I remember they always used to use mouth caps filled with fake blood and they'd smear it on their face. Roman must have done the same.

just like Undertaker and Kane are the only wrestlers Vince allows to perform the tombstone move, Brock is probably the only one allowed to legitimately bust people open like that.
it's basically just slapping/scraping an elbow across the top of the head to cause a tear in the skin.... easier said than done though (without legit hurting someone)
