The problem with this questionnaire is that the questions are all biased towards women. It hits all off the areas that women care about and doesn't reflect the things that men care about.
Sending a man a questionnaire like this turns into a one way interview when dating is about two people getting to know each other. If a suitor answers these questions and they're not to the women's liking, that's it. It's over. Without the man being able to equally question the woman, let alone from a man's perspective.
It's an unequal questionnaire and gives women more power. When in reality, the end goal of dating is marriage. A woman marries into a man's family, not the other way around. The man should be questioning whether a woman is compatible for him and his family.
