I guess she wasn't super obese to get the surgical cut and just got liposuction.
From what I've seen in the extreme makeover show, if there's too much stretched skin and fat in the abdominal area, then they will make a curved cut from hip-to-hip that looks like a crecent shape to remove excess flab and they sew it up. If they didn't do this, you would have a skinny person with a six-pack and a layer of flab hanging over the front of the bikini.
