People die from falling off ladders all the time when they're only a few feet off the ground. When people slip and fall in a way they can't brace themselves, even a fall from flat ground can be deadly. 40 feet is much higher than it sounds.

Real talk. I fell backward from standing height back of my head hit glass coffee table, worst pain ever felt and that was enough to give me a hemorrhage into my brain. Not surprised of the outcome from an unexpected 40ft fall.
