The incident happened when he was being moved back into the prison block after spending time in segregation

This is the third incident Hernandez has been involved in since he has been at the Souza Baranowski Correctional Facility in Shirley.

Hernandez was the look out for a fight between inmates.

He was also caught smoking in the prison yard.
