I can’t stress enough how important premarital counseling is especially for people who do not come from a line of successful marriages. It will help reset expectations. I just had a pregnant patient last week with HIV. At least here in NY, you cannot disclose it to anyone. We do have to notify the department of health, which then sends a letter to the person exposed ( in this case the husband) that someone he’s been with has HIV.

Frankly, this sucks. Because in this case we told the patient that he would be receiving this letter, giving her the heads up. And now that she knows, what is gonna stop her from throwing that mail away when it arrives?
