When they do the surgery for breast implants they will either remove the areola and insert the implant through there or
Make an incision under the breast to hide the scarring
Looks like hers went through the nipple section and then sew it back on
Like putting a lid on a jar but the size of the implant is greater than the previous area
the scarring from removing the nipple is around the outer area so they did something to hide the scars
Maybe used lasers or something to intentionally scar it so it blends in and just looks like areola
