Barksdale ran the prison, so they probably figured he was involved and opted not to investigate it any further.
D Angelo was in a strange situation but he could’ve at least accepted whatever help Avon was giving considering they were in the same damn prison.

Power. I think Avon just thought no one would be stupid enough to kill D, knowing he's Avon's nephew, so he must've killed himself (even though it was the most unrealistic way).
