He wasn't a great freethrow shooter but the base he had to work with was much better than this, all he needed to do with his old form was to speed it up and remove the hitch and it would have been fine. If he's injured then he shouldn't be playing, if a shooting coach changed his form to that they need to be fired because either situation is affecting the confidence he has in his jumper.

The form on his jumpshot was sound in July and he was capable of hitting threes off the dribble like he did in College, and now he wont even shoot them.

He got fukked over by the player development crew. They tried to change his jumper, canned the changes, and now want him to go back to his old shot. Now he's just mentally screwed - he's playing scared and is over thinking things. There are times when in live action he just goes up for his shot and it looks fine. Give him time to think about it though and it all goes to hell.
