Some people are gifted. I wouldn't be surprised if the artist was left handed. People that are left handed or ambidextrous tend to be natural artists.

Years of practice and learning art techniques. Like @1984 said people will sometimes use grids as well as other techniques. I personally freehand everything though

Part of it is training in art classes, you learn from artists. But to do photo-realism, a lot of that is a natural talent. My wife's friend's daughter is 15 and can do photo-realism drawings using pencil on paper. Mind blowing talent. I can draw, but nothing on that level.
