How does this happen?
how does this happen :ohhh:

Lymph nodes are plugged up and ain't draining for some reason. So the lymphatic fluid just keeps building up. It can be genetic or due to damage like cancer treatments, infection, or injury.

Is there any way she can get a fat transfer or fat removal for that leg?
It's not fat, it's fluid, and the fluid is so evenly distributed that there's no real effective way to drain it. There are various treatments to control it as best you can.

Ironically some people DO get surgery to remove fat from the leg, cause so long as you can't control the fluid the least you can do is get as much fat out of that leg as possible and that does reduce the mass and bulk of the leg somewhat.
