He is a grown man, full mustache and beard, and even if he is mentally challenged he still has the full on power of a fully developed male. So a female feeling worried that he is in a space reserved for females seems perfectly logical and reasonable to me.

An adult male accompanied by his mother in a women's restroom does not change the fact that it is still a man in a women's restroom.
