Haven't needed to come up with an excuse for some time; but my best one was:
Someone smashed the windshield on my car.
No Doctor's bullshyt, no "well just come in later", didn't have to lie about a family member dying...worked perfectly.

I used to keep a picture of a flat tire on my phone then send it to my manager like "caught a flat, far from work, won't be coming in"
