When advertisers create a campaign they take into consideration that only a very small percentage of the people that see an ad will actually buy the product/service, what matters is the penetration rate of the image, more looks it has means more potential $.
Agencies create images that make some kind of impact (perception of negative/positive doesn't matter to a certain degree) as long as it reaches the widest band.
