I personally feel they knew Charlotte was coming back heel, they just thought the crowd wouldn't be so brutal to her since she was coming back from such a rough injury. Even I was shocked the booed her that hard, but I think everyone involved had it set up where Tiff is gonna be the face and Charlotte the heel.

Jade wanted to test Bianca's loyalty so she damaged her face and knocked herself out hoping that she wouldn't merely be replaced. When she saw how easily Bianca replaced her with Naomi she knew that the friendship wasn't real.
