with your hypothetical regulation scenario they would be just as bad as any of the bottom teams. regulated teams are just as bad a promoted teams.??? That wouldn't happen. The Mad Ants are in the G-League.
Do the Columbus Clippers play the Yankees?? No. That's not how promotion/relegation would work.
the game is free flowing now
i.e. that ugly thing y'all call defense is kept to a minimum

there isn't a plethora of lebron's, harden's, westbrook's, durant's, curry's, etc in the world. those 10 new teams would be comprised of a bunch of mediocre players.
for a relgation system to take place you would basically have to take away the draft and even the salary cap like soccer and that would just make the rich stay rich and poor stay poor like in the premier league where the manchester united's and chelsea's of the world are always better than ther huddersfeild's and bournemouth's of the world.
Defense in general is played only in segments. It used to be offensive to give up 100 a night. Now you can give up 110 in a game and be like "yeah we got key stops and played solidly on that end."
High level defense is still played, stop with the silly narrative that there is no defense.
baseball doesn't have a relegation system either. Then don't take away the draft and salary cap. I don't see how promotion/relegation couldn't work with a salary cap and draft. Baseball doesn't have a salary cap, it still has a draft.
The reasons behind the disparities in european soccer are not from promotion/relegation and they don't have drafts/caps because they have player academies and sign 11 year olds.
As for talent, a league of 20 teams is better for talent distribution than one of 30. The league right now a pseudo-relegation now with how the eastern conference and western conference are split.
