I feel where you're coming from, but the problem (at least to me) is that nothing can affect your looks like morbid obesity. It's not like she has a scar or is missing a tooth where we can imagine how she would look like without that ailment. None of us have any idea how she would look if she were to slim down a bit.
Hell I don't even think she has an idea how she might look if she was able to lose some weight.
She might not be ugly at all, but that weight is such a hurdle to get over
