If he was able to take Finasteride, that would have dramatically slowed down (maybe even stopped) his hair loss. The crown when thinning is easier to grow back than the hairline. But he’s slick bald up there now. A transplant may be the only thing that fixes it.

For a thinning crown there are two options.
Finasteride which is an anti-androgen that stops DHT from attacking your scalp. Scalp hair has varying sensitivity to DHT depending on the person. That’s why some people for bald and others don’t.
Minoxidil can help regrow thinning hair on the scalp. It’s a temporary fix and it you stop it you will lose all of your results in a shock type loss. That’s because although you’re growing the hair, the hair loss process is still going on.
For a slick bald spot like KD has, a transplant is really the best option for that although Finasteride and Minoxidil may work.
