People overrate intelligence in certain areas and think it applies everywhere. She can retain info and understanding how to attack illness and has excellent hand eye coordination. That has nothing to do with people/relationship stuff. And she has spent her life in school and training so she is probably sheltered and detached and probably even awkward socially.

She said she’s making that in her 1st year and she’ll be making $500K next year. Sounds like she recently finished up her residency and fellowship. She is probably just starting to make real money now and the discrepancy is becoming more apparent.

They’re usually the top earners making well over 300k easy. At that income level there are strategies available to them that could benefit him and her in the long run while he pursues his acting career in the meantime. Working a part time job will only kick the can down the road if you will without a solid plan for income stability. If he could prioritize his time and be efficient he could take advantage of his new found free time and produce a source of income that could grow larger than what she makes.
