Have any of it's security/privacy protection claims come into question recently? If so, what?
its a distro but its specifically designed to make you anonymous and protect privacy. I still wouldn't use it though, i aint got shyt to hide like that.

Why wouldn't you use it?
Also, for the protections it provides does the user need to use it from a computer that has never been compromised

It's "protections" are all open source/popular programs (tor, pgp, OTR, etc) that you could use under your normal OS. These programs haven't particularly come under fire but they do have certain limitations.
Tails is a live distribution (distro - Linux distribution based off Debian). It's main selling point is that it never caches/saves anything to your physical machine making data recovery next to impossible (what they call amnesic) along with bundling relevant personal security tools in a neat package that can be ran on virtually any machine.
