Most people literally don't think before they speak/act. There is no voice in their head that talks things through beforehand.

literally nothing goes on up there. they just react to stimuli and if they "think" anything it's a visual.

you might think, "i want a coffee". They will just "see" a coffee in their brain and know they want coffee
