It looks like the Botswana runner clipped the Belgian runner with the baton that's why it flipped up.

Usually a lack of concentration. Or sometimes the outgoing runner goes off too early and the incoming runner starts to slow down to heavy legs. If you slow the exchange down it phucks up the timing
