It could've been like a mistake, somewhere in the neighborhood of leaving her in the tub alone due to a distraction. That's quite common. Cats can get in a baby's crib and sit on their face which leads to suffocation. Baby monitor can't warn you of that obviously. Has to be something unintentionally that she feels she could have prevented. Idk though.
