I'm going with Britt Baker (on behalf of Cole) as The Devil with Fish, O'Reilly, and Wardlow as the masked goons.

...and then when the goons get revealed and they've got an injured Cole and MJF surrounded, Cole turns on MJF to reveal that he's been healthy and in on it all along.

That's when the devil gets unmasked as Britt and we eventually deduce that they've both been playing the part.
