I think a lot men don't understand/care than women's bodies are completely different than theirs. Sex is not visual for us; it's mental.We need to be mentally stimulated. Flirting, touching, dirty talk, assertiveness etc are all things we really enjoy. There is a reason women love romance novels.
I think some men don't want to change it up b/c of pride. I once asked an ex would be pull my hair and his exact response was "No, I'm not going to have sex with you the way another man had sex with you."
If he is doing a great job in the bedroom she won't turn him down.

She wasn't that attracted to him, and the sex was never good to begin with. Doesn't feel she should have to put up with it or fake enjoyment anymore because she does other important things to contribute to the marriage.
Typical woman justifications. Men can do all these things you've mentioned to cater or please his wife mentally &/or physically to stimulate their sex life & in most cases she'll still be frigid or come up with some unsatisfactory excuse to deny him sex on a regular basis.
