I remember being on site and a Crane Operator kept the steel ball brake line tight as he was lifting something and the line snapped and they ball shot off and hit a gas line. Could have killed everyone if if exploded.

Don’t trust that people are paying attention. Never ever go underneath equipment unless it’s locked out.
