I've interacted with a decent amount working pharmacy and almost all are on medicaid and even then they have to jump through hoops to get their meds paid for particularly estrogen and testosterone as when you submit the claims the insurance immediately flags the sex unless they and their doctor have done all the paperwork to get it covered.

I can't imagine a significant amount of transgender women having access to something of this magnitude and level of sophistication. If this is occurring at all it's extremely rare.
