They had toilets like that in Tanzania/Kenya, they was outside without a roof, and you basically couldn't use the toilets during the night or when it was raining.

they dont have to. Some toilet seats based on their size, help with your form. Other wise, you can put something under your feet to lift them b/c proper form needs to have knees above or slightly higher than your hips, sit up with your back straight and stomach puffed out out slightly.
