In my experiences in Japan, they go out their way to make you feel comfortable.
In the big cities and major highways, all the signs are in both English and Japanese.
For restaurants, most have English menus or pictures where you can just point to what you want and in the rare times neither were available the chef would just draw a picture of a pig, chicken or cow when ordering at a yakitori place for example
When just walking around I have been approached randomly by the locals just so they can test their English
