A 28week old baby is very fragile. While something like this is very unfortunate, the fact that this happened is not completely outside the realm of possibility. Those calling for the doctors disbarment over this incident need to relax. You cant simply disbar a physician for just one isolated incident.

I think the Dr. was getting impatient. The head cleared and he just started pulling instead of trying to clear the shoulders before pulling the head.
