Now, I'm a spiritual dude with a personal relationship with the Higher Power but there's no way to classify my nuanced spiritual identity. I was raised Catholic so once I confirmed the hospital was indeed Catholic that's what I registered as.
Now if it were a Baptist or a Methodist hospital I would have answered accordingly. Like Hell if I'm going to be put in the non-believer suite.
Ask yourself this: Would you be likely to be treated better or worse if you classified yourself as Muslim?
I'm telling you, these questions and categories have no place in medicine. None. Hell, they have no place in greater social life. It's got to stop. It will once we stop participating.
Can you imagine if all Black people checked "white" on the next census? Can you imagine the social disruption that would cause? The progress?
Suddenly our property values would spike and they'd actually extend city services (other than Gestapo-style policing) to our neighborhoods.
