I think it meant he continued to kill even after he got out of prison. Because he wasn't sure if he killed the other guy. But I dunno. Like you said, shyt was weird. If they left off that last scene the movie is damn near perfect.

That's actually a good theory.
I dunno, fukk it. I don't like ambiguous ass endings to movies or anything really. Especially when it's tacked on at the last second and doesn't really add anything to the story.

I think the movie makes sense and follows the pattern.
If you notice he always said he was a killer. He also has dementia.
When does it state/say he was released? He still was suspected for past murders. He was also so far gone (dementia progressed) that he called Eun-hee "Sis."
The necklace is what gave this away. Remember he was peeling off Eun-hee when the recorder told him the real. He must have put the cop's pic there to remind him. The end may just have been his imagination that he was let out the psych ward. And since he always said he was a killer, he needed a purpose to continue on, and Tae-joo was his degenerate mental motivator.
