Seriously implement these things (after considering professional medical advice about them ofcourse) :

1.) If you are masturbating or ejaculating fequently.........STOP. That kind of frequent stimulation 'shocks' your nervous system & gradually numbs it. Also it litterally mutates your brain and thus conditions you to be lethargic & constantly 'seeking' novelty resulting in low attention span - and attention is the foundatioin of good memory.

2.) How is your diet? Reduce your SUGAR INTAKE ----- Also EAT MORE fish like salmon, and nuts like WALNUTS, Pistacios and cashews - in a word, HEALTHY FATS. The omega 3 fatty acid content of those foods are supposedly essential for a healthy brain.

4.) Take supplements. Such as Omega 3 fish oil, Iron and or magnessium. THESE ARE PROBABLY THE MOST IMPORTANT nutrients you need for every thing to function right in your body.

5.) Exercise / get movin, it stimulates blood flow

4.) Meditation. Take some time out of your day to just relax and breath deeply from your abdomen. - This works wonders.

A B12 pill, 1000 units, will improve your concentration in about an hour.

Fatigue is probably due to low vitamin D. There is no dosage that is right for everyone. Some need more, others need less. Try 5000 units a day for five days.

Forgetting your sister is normal since you only give her a ride once a week.
