You don't put down their details on the CV, just write "Available upon request"

They'll contact them after you've gotten the job and accepted the offer. Yes they have to be real people and you also have to tell them how you know them/they know you.

Not all jobs asks for or check references.
Why would they contact them after you have the job? Defeats the purpose no?

Personal opinion, unless it's going to put you in a bad position at your current job I would include them. It's one less item they have to follow-up with you on and they might know someone you list helping you to the front of the line.

When they want references they will ask for them at the interview which signifies your a strong candidate for the job if they want to get feedback from real people who you have worked for...always leave places on good terms for that purpose

go and visit your old manager and tell him the situation if he can do you a solid and make a nice narrative for you and that you left to go to school or something and that you were a gracious worker who always tried his best

I have added responsiblities or made what I did seem more than what it was but I would explain that to my old boss before I would use him as a reference and he was cool with it and went along with it in case they asked
