remember when the refs back would be turned, and a heel would reach into their tights and pull out a foreign object(maybe a roll of hard tape or brass knucks), clock the opponent, then hurriedly stuff back in their tights while the ref turns around and counts for the pinfall. 
Or a manager like Mr Fuji would distract the ref, slide his cane into the ring under the bottom rope and Demolition would hit the opponent with it then throw it out the ring and get the pinfall.
Or like when Andre the Giant was the Million Dollar Man's bodyguard at WM 4, and he knocked the sh*t outta Hacksaw when the ref wasn't looking and Dibiase got the pin.
This is tried & true character building 101. Everyone saw what happened except for the ref, which makes it more infuriating to the crowd. It's apart of the fabric of wrestling no matter what era it is. It's like they want ppl to be a heel but still be liked.
